The April playtest reportedly centered on Jill Valentine navigating an abandoned hospital on an island. Players had to search for a serum to combat a mold infection while being pursued by an unkillable stalker enemy called "Goatman." Chris Redfield and Barry Burton assisted Jill via radio. The playtest ended with Goatman catching and infecting Jill.
The September playtest focused on Leon Kennedy exploring a beach and forest near "Mr. Simon's lab." Leon's objective involved fighting weapon-wielding zombies and avoiding another unkillable stalker, a witch capable of transforming into flies and inducing hallucinations. The playtest also featured zombies praying to the moon, potentially linking to a cult symbolized by a moon sigil seen in Jill's story.
