Obtaining the Sacrificial Heart
You acquire the Sacrificial Heart by defeating enemies in Utzaal, which is in act three. It's not immediately obvious where to use it after that.
How to Use the Sacrificial Heart
You need to take the heart to the Sacrificial Dais in Aggorat. This area is accessible immediately after Utzaal by defeating the Viper Napuatzi boss. Proceed through the city until you reach the Trial of Sacrifice section. Within that area, locate a checkpoint, and the Sacrificial Dais is directly after it. You'll find it marked as "Blood Sacrifice" on your map.
If you arrive at The Black Chambers, you've gone too far. Interact with the Sacrificial Dais, grab the Sacrificial Dagger, place the heart on the altar, and stab it to receive two passive skill points and two weapon set skill points.
What if You've Progressed to Act One (Cruel)?
If you have completed act three and opened the portal to act one (cruel), it can be challenging to return to Aggorat. The best way to get back is by using the waypoint in act one (cruel)'s Clearfell Encampment. Activating this waypoint will give you an option to travel back to Aggorat, by selecting act three and then selecting the relevant waypoint.
